Medical records are processed and housed in the Health Information Services Department, which is located on the B1 level near the Medical Library. Please contact the HIS general number (listed below) to have your charts pulled for review and completion. Dictation numbers are obtained through Paula Anderson in the Transcription Department at 879-3352. It is important that you be sure to use the correct numbers when using the dictation system in order to get your documents transcribed timely and appropriately. H&P's and operative reports need to be completed within 24 hours. Remember to provide a handwritten brief operative note and h&p to be placed on the chart pending completion of your dictation. Verbal and telephone orders must be signed, dated and timed within 72 hours - you may sign for your partners. Also, you are reminded to write progress notes daily. The entire record must be completed within 30 days of the patient's discharge. Medical record requirements, provisions for administrative withdrawal of privileges for incomplete records, and penalties for exercising privileges when on administrative withdrawal of privileges are described in detail in Section 5 of the Medical Staff Rules and Regulations.
