Physician Handbook

Mercy employs a dedicated team of Care Managers who, with your medical direction, coordinate the discharge process.  Care Managers also assure that we meet the quality care needs of patients and their families, along with the requirements of our payors.  The Care Managers assist with the timely provision of available beds for our acutely ill patients.  They enjoy a strong collaborative relationship with the Medical Staff.

The Care Managers provide consultation to health care team members on clinical issues concerning patient care along the continuum.  They also provide proactive identification of utilization review and potential risk management issues occurring in the clinical areas, by collecting data and communicating findings to the appropriate departments.
