Our Services and Programs
From monitoring the complications of diabetes, to supporting rehabilitation after surgery, to helping families cope with a terminal illness, VNA Home Health Care has a whole team of professionals who bring compassionate and clinically excellent care into the home. Working in collaboration with your primary care physician, we will develop a comprehensive plan of care developed specifically to fit your needs.

It all begins with an initial assessment, which you, a friend or family member, can request. We'll meet with you to discuss your situation, decide the level of care you need and determine what outside resources - from your family to agencies in the community - are available to help you.

Our Team Includes:

    Skilled Nurses
    A full range of nursing services including wound care, IV therapy, diabetic care, catheter care, cardiac and respiratory assessment, ostomy and continence evaluation and treatment, medication management, pain management, and cancer care.

    Therapists
    Assessment and treatment by physical therapists, occupational therapists and speech language pathologists for adults and children.

    Medical Social Workers
    Case management, grief counseling, coordination of community resources, and assistance with social/emotional effects of acute and chronic illness.

    Home Health Aides
    Assistance with daily living activities and personal care.

    Healing at Home
    We work as a team to provide services for individuals with acute or exacerbation of illness, with an emphasis on returning to independence.

    Rehabilitation
    Treatment and instruction by skilled therapists on home safety, balance, orthopedic rehabilitation, cognitive problems and restorative programming.

    Mental Health
    Services for adults, adolescents, and children with emotional problems, substance abuse or mental illness.

    Hospice
    Comfort and dignity in the final stage of life - supporting the physical, spiritual, and emotional needs of the individual and family members.

    Community Wellness
    We use prevention and education to play a crucial role in keeping people healthy and safe, we sponsor a wide range of clinics and services for both businesses and community - including education, screening, testing, and immunizations - designed to make healthcare easy and accessible to all.
Rehabilitation Services
    Physical Therapy
    Our Physical Therapists assess and provide treatment to help patients of all ages increase mobility, strength, balance, and coordination. Therapists also work with family and community resources to maximize the patient's rehabilitation process.

    Occupational Therapy
    Occupational Therapists assess and treat patients in order to increase their self-help and homemaking skills. Therapists help those who have suffered physical impairment to return to their maximal level of independence.

    Speech-Language Therapy
    Our Speech-Language Therapists help patients with their language and speech skills so they may regain control of their communication skills. They also teach patients and family members safe and appropriate swallowing techniques.

    Pediatric Therapy
    Our Physical, Speech, and Occupational Therapists evaluate and treat developmental delays in children ages 0 through 6. They work closely with community child development services.
